 Damage Report Form

When damage occurs from an event such as a tornado, severe weather outbreaks, earthquakes or winter storms, information is gathered by the Jefferson County Emergency Management Agency from a variety of sources to determine the magnitude of damage. This includes information from individuals, local governments, county departments and agencies, public safety entities and telephone reports that are made directly to the EMA.

This damage report form is mechanism for the public to report damage within Jefferson County 24-hours a day, 7 days a week. If you have damage such as trees down, power lines down, structure damage, etc. to report, please fill out the form below completely. Someone from the Jefferson County EMA will be in contact with you to ask follow-up questions.
